Case Management is a recovery-based service that helps individuals access the resources they need to live independently in the community. Case managers can also advocate for people and help them navigate the system. They assess people's needs, develop a plan of care, and connect them with the services they need.

Case managers are available to adults, adolescents, and children who have difficulty accessing community resources or arranging services for themselves. The goal of case management is to help people achieve their recovery goals and live as independently as possible.

Specifically, case managers can help with:
-
Maintaining eligibility for benefits and services
-
Completing paperwork and forms
-
Finding housing and financial resources
-
Connecting with vocational or educational opportunities
-
Building social and interpersonal supports
-
Navigating the health care system
Case management is a valuable resource for people who need help accessing the resources they need to live independently in the community. If you or someone you know is interested in learning more about case management, please contact your local mental health or social services agency.

Peer to Peer support is available for individuals and their families. Our services are non-authoritative and peer-centered, and we use evidence-based tools to help people achieve recovery.

Our support specialists are trained to relate and support, not to instruct. They have been vetted and evaluated for their own experiences with psychotherapy.
